FETAL MACROSOMIA

Many different definitions have been proposed. All definitions, however, fail to differentiate the genetically large infant, from the infant who is large due a pathological or pathophysiological process.

  1. The best definitions are probably based on population based centile charts: 90th, 95th or 97th percentile.
  2. Birth weight >4000grams, >4200 grams, or >4500 grams.

The most accurate diagnosis is probably an expected fetal weight of >4200 grams and changes in fetal morphometry (increase in abdominal circumference with respect to head circumference and femur length).

MATERNAL RISK FACTORS

  1. Maternal diabetes mellitus (both gestational and insulin dependent).
  2. Maternal obesity, total weight gain during pregnancy, maternal height
  3. Post dates.
